1. Copy of duly filled application form of Karnataka NEET MDS 2024 counseling.
2. Two passport-sized photographs
3. Valid photo ID proof (PAN card/driving license/voter ID/passport/Aadhar card/ration card)
4. NEET MDS 2024 admit card.
5. NEET MDS 2024 scorecard
6. Class X marksheet
7. Class XII marksheet
8. MBBS marksheets
9. Qualifying degree certificate
10. Internship completion certificate
11. Certificate from the head of the institution certifying that the medical college is recognized by DCI
12. Eligibility Certificate (for candidates who have graduated from an institute other than RGUHS)
13. Migration Certificate (for candidates who have graduated from an institute other than RGUHS)
14. Registration Certificate issued by MCI/SMC
15. Compulsory Government service bond on a stamp paper of INR 100 (Annexure 1)
16. Undertaking on a stamp paper of INR 200, stating that the candidate will serve under the state government of Karnataka for a period of three years
17. Undertaking on a stamp paper of INR 20, for completion of the document verification process
18. Caste/community certificate (if applicable)
